Provision Surgical: Transforming Medical Imaging into Real-time Visualization and Precise ExecutionΒ
Welcome to the Provision Surgical Q4 2026 Update

Dear Stakeholders and Interested Parties,
Recent months have been defined by momentum; an AWS AI Pitch Competition win and being backed by LvlUp Ventures, and real progress on PAIR Guide alongside our growing regulatory and academic partnerships. Each milestone reflects growing confidence in the problem we're solving and the approach we're taking.
As we look ahead to Q4 2026, we remain focused on completing development, advancing our regulatory pathway, and deepening clinical partnerships. Thank you for being part of this journey.

Sincerely,
Abdullah Bahour, CEO/CTO
Over the past three quarters, we've kept building; advancing the PAIR Guide platform, showing up at conferences across North America, and pushing toward a strong finish to 2026. Here's a look at what's been happening.
We've expanded the PAIR Guide head mounted displays to the XREAL Pro AR glasses, marking a major step forward in how surgeons will interact with the platform. This move brings a lighter, more streamlined hardware experience to the OR, allowing surgeons to access real-time 3D guidanceΒ in a sleeker, more surgeon-friendly form factor. The switch reflects our continued focus on making PAIR Guide as intuitive and unobtrusive as possible at the point of care.

PAIR Guide gives surgeons real-time 3D vision inside the operating room, projecting patient-specific anatomy and surgical plans directly onto the surgical site, with no robot required. The system transforms preoperative 2D imaging into live, actionable 3D guidance at the point of care. This quarter, we also advanced our patented preoperative planning software, which combines AI with the functions of 10+ industry tools into a single platform; processing each patient's CT/MRI scans into a case-specific bundle of bone models, plans, and images in under 10 minutes, turning imaging data into a ready-to-use surgical plan before the patient ever reaches the OR.

Provision Surgical has been backed by LvlUp Ventures' support as we continue scaling toward our next milestones.

We are proud to celebrate our Co-Founder and Chief Medical Officer, Dr. Joyce Wang, on her graduation with an MD/MBA dual degree from Georgetown University and on beginning her Orthopaedic Surgery residency at the University of Virginia. Her perspective as both a clinician and entrepreneur has been instrumental in guiding the development of PAIR Guide.

Provision Surgical is pleased to welcome RAi by Clemtech Medical as our regulatory consulting partner. RAi is an AI-powered regulatory operating system built for FDA-regulated medical device companies; covering submissions, QMS, CAPA, risk management, audit prep, and supplier control in one platform. This partnership strengthens our regulatory infrastructure as we advance toward our FDA 510(k) submission, bringing expert guidance and purpose-built tooling to our compliance and quality process.

Provision Surgical's IP portfolio continues to grow, protecting core innovations and reinforcing long-term differentiation as we advance toward commercialization.

Provision Surgical is preparing to launch Provision Academy; an AR-native pre-operative planning platform built for medical education. Designed for medical schools, simulation centers, and orthopaedic residency programs, Provision Academy advances surgical training through augmented reality.
The platform covers three core procedures: Total Knee Arthroplasty (TKA), High Tibial Osteotomy (HTO), and ACL Reconstruction; giving students a hands-on spatial relationship with anatomy, alignment, and instrumentation before they ever enter the OR. Students plan on desktop, then experience the same plan spatially on a headset, paired via a simple 6-digit code.
Provision Academy is designed to complement PAIR Guide's clinical pathway; building the next generation of surgeons who are fluent in spatial anatomy from day one.
Provision Surgical competed at the AWS AI Pitch Competition and walked away with first place, earning $50,000 in AWS Partner-funded resources to support development and implementation.
Hosted in partnership with Tennessee SBDC, nClouds, PREDICTif Solutions, and Southwest Tennessee Community College. This win is a strong validation of what we are building. PAIR Guide is solving a real problem in the OR, and this recognition tells us we are headed in exactly the right direction

Provision Surgical was active on the conference circuit this period, attending UMMAHCON in Toronto; a space connecting founders, builders, and investors during Toronto Tech Week and the MBSA National Conference in Chicago. Both events offered valuable opportunities to engage with founders, investors, and healthcare builders shaping the next generation of surgical innovation.
As we move into the fourth quarter of 2026, Provision Surgical is focused on:
π PAIR Guide development Completing the build and advancing toward FDA 510(k) submission.
π₯ Pilot user testing Working with clinical partners to validate the system in real surgical environments.
πΌ Navigational tower integration Finalizing hardware integration for OR-ready deployment.
π Provision Academy launch Bringing our AR surgical education platform to medical schools, simulation centers, and residency programs.
π International patents Filing to expand and protect our IP portfolio globally.
π€Clinical and strategic partnerships Deepening engagement with clinical sites and strategic collaborators.
We welcome partners, collaborators, and supporters as we continue building toward our next phase. We are opening conversations with aligned investors interested in learning more about our progress and what's ahead.
